PHOTO, VIDEO & MEDIA RELEASE

By allowing your child to participate in the Future Ready Leadership & Life Skills Program, you grant permission for ChiGlobal Consulting LLC, Howard County, and their partners to photograph, videotape, and audio record program activities for educational, promotional, marketing, reporting, and program documentation purposes.

Photos, videos, testimonials, student work products, and recordings may be used in program reports, social media, websites, promotional materials, presentations, and grant & sponsor reporting.

​No personal contact information will be shared publicly.

In the case that we choose to use your child’s information, we will alert you.

If you do not wish for your child to be photographed or recorded, please notify us in writing before the start of the program.

RISK ACKNOWLEDGMENT & LIABILITY WAIVER

Parents/guardians acknowledge that participation in Future Ready! activities may involve physical movement, team-building exercises, field experiences, and other supervised activities. By participating, parents/guardians agree that:

1. Students participate voluntarily.
2. Students will follow program expectations and staff instructions.
3. ChiGlobal Consulting LLC, Howard County, staff, volunteers, partners, and sponsors will not be held liable for injuries resulting from ordinary participation in program activities, except as required by law.
